Please return this form by Tuesday 10 December with payment to secure a place for your child.

Please note up to Tuesday 10 December  priority is given to existing music instrument students.  After Tuesday places will be allocated on a first come first served basis.  To secure the same time for after school lessons with any teacher please ensure you return your form with payment by this date.
